Proposed Action
Staff recommends adoption of the following motion: Move to approve a resolution authorizing
employee dental insurance rates for 2013.
Passage of this motion will allow the City to continue to offer dental insurance through
HealthPartners.

Employee Dental Insurance Rates for 2013
Item No.
The City has been enrolled in the HealthPartners dental plan since 2009. The rates for 2013 are
not proposed to increase.
The City currently pays 100% of the cost of individual coverage for employees. If employees
choose to enroll their dependents, they are responsible for 100% of the additional dependent
coverage. Staff is recommending that we continue this cost sharing method.
Primary Issues to Consider
• Is there any financial impact?
The Police Officers' contract has not yet been settled for 2012 -2013. However, there is no
proposed change in the rate structure; therefore, no financial impact is anticipated.

RESOLUTION AUTHORIZING
EMPLOYEE DENTAL INSURANCE RATES FOR 2013
WHEREAS, the City Council will establish from time to time the insurance plans
that will be provided; and
WHEREAS, the City Council will establish the costs the City will pay for
employee and dependent coverage under City group dental insurance programs;
NOW THEREFORE, BE IT RESOLVED by the City Council of the City of
Lakeville that the following monthly cost responsibility be adopted for the
HealthPartners Open Access Dental Choice insurance plan effective January 1, 2013:
APPROVED and ADOPTED by the City Council of the City of Lakeville this
5th day of November 2012.
