The information must be submitted to the City of Lakeville Administration Department. Plan Review: Upon receipt of the information the City will: Determine if a structural analysis of the monopole or water tower needs to be completed. If so, the plans will be sent to Ulteig to obtain a cost estimate for a structural analysis and to KLM Engineering for a cost estimate to performing the plan review and inspection services The estimates will be provided to cellular company representatives and an escrow to cover the costs of the services provided by Ulteig and KLM must be submitted to the City prior to The City will also determine if: agreement Any Planning Department permits or approvals are required (Planning actions typically not required unless it exceeds approved antenna height) Review Fees: The cellular company or its representatives must provide payment in advance for the following: Ulteig and KLM consultant services as listed above City administration fee Attorney fee (only if a lease amendment is required) Authorization to Proceed: The City will issue an authorization to proceed to the cellular company or its representatives: After receipt of approved plans and structural analysis After approval of a lease amendment (if required) Determined by Engineering Firm Determined by Engineering Firm Determined by Engineering Firm Cost $425.00 $500.00 NOTE: authorization to proceed is valid for 120 days or until the next carrier requests modification which may affect the results of the structural analysis — whichever is later.
