HomeMy WebLinkAbout78-008Date: 6 February, 1978 Motion by Zaun CITY OF LAKEVILLE RESOLUTION Resolution No. 78-8 Seconded by Nelson RESOLUTION SELF-INSURING WEEKLY INDEMNITY WHEREAS, weekly indemnity benefits were provided by the Banker's Life Insurance Company of Iowa for City employees prior to February 15, 1978. NOW THEREFORE, BE IT RESOLVED, the City will hereby deposit $3,500 into the Payroll Benefit Fund from the General Fund to be used exclusively for payment of weekly indemnity claims as described below. The City shall maintain a minimum reserve of $55 per employee in the Payroll Benefit Fund at all times. ELIGIBILITY REQUIREMENTS AND CONDITIONS 1. Weekly income benefits of $55 per week for up to 26 weeks will be paid to full time employees for loss of working time, due to sick- ness or injury. To qualify, the employee must be wholly and con- tinuously disabled because of an injury or sickness, unable to perform duties of the occupation and not engaged in any other occupation for wage or profit. The employee must.also be under the care of a physician, and provide letter on the disability. 2. Benefits will be paid beginning with the first day of an injury and the fifteenth day of a sickness. 3. Two or more periods of disability due to the same cause are con- sidered one period of disability unless they are separated by an employees return to full time work. Two or more periods of dis- ability due to an unrelated injury or sickness are considered one period of disability unless separated by an employees return to full time work. 4. Disability will be considered due to sickness unless disability is the direct result of and commences within thirty days after an accidental bodily injury. 5. Weekly income benefits covers the employee for disabilities due to injuries, other than injuries arising out of, or in the course of, any employment for wage or profit. 6. Reference to "full time employee" or "employee" is defined as a person who works more than an average of 35 hours per week. It does not include those who are hired as Consultants. 7. In no case shall an employee be paid more than what they would have received had they worked a normal work week. If an employee is receiving sick leave pay, the weekly income benefit will re- place, dollar for dollar, the wages received for sick pay,.. up to $55 per week. ADOPTED this 6th day of February 1978.
