Home My WebLink About77-50CITY OF LAKEVILLE RESOLUTION Date: 1 August, 1977 Resolution No. 77-50 Motion by Emond Seconded by Zaun AMBULANCE SERVICE CHARGE RATES WHEREAS, the City Councils of the Cities of Apple Valley, Lakeville and Farmington, (ALF) did on the 30th day of December, 1975, the 5th day of January, 1976, and the 19th day of January, 1976, respectively, did approve an agreement with Midwest Med-Kab, Inc., d/b/a/ Dakota Ambulance Service, effective January 1, 1976, and continuing through December 31, 1977, for pro- viding of ambulance services with a municipal subsidy, and WHEREAS, Exhibit B of said agreement established the rates to be charged by Dakota Ambulance Service, and WHEREAS, Dakota Ambulance Service in a letter dated June 9, 1977, a copy of which is attached to this Resolution, did request change in certain line items of said Exhibit B rate schedule, and WHEREAS, the agreement above referenced between ALF and Dakota Ambulance Service provides that the Exhibit B rate schedule can be m:;dified with approval of ALF after October 1, 1976, and WHEREAS, said rate schedule has not heretofore been amended. NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Lakeville as follows: 1. That the rate schedule referred to as Exhibit B in that cer- tain agreement referred to above between ALF and Dakota Ambu- lance Service is hereby approved to be amended as described in the letter from Dakota Ambulance Service dated June 9, 1977, which is attached to and made a part of this Resolution. 2. That the changes in said Exhibit B rate schedule shall be null and void unless this Resolution is adopted by each City Council in the Cities of Apple Valley, Lakeville and Farmington. 3. That this Resolution shall in no way modify the Resolutions adopted by the ALF City Council's on August 25, 1976, Septem- ber 7, 1976, and September 20, 1976, as it relates to sur- charges for servicebeyond.the municipal boundaries of ALF.
