RESOLUTION NO. 21-132
Resolution Adopting Voluntary COVID-19 Vaccination Incentive Program
WHEREAS, the City of Lakeville is committed to providing a safe and healthy workplace for all
our employees; and
WHEREAS, the voluntary COVID-19 vaccination incentive program will encourage employees
to become fully vaccinated against the COVID-19 virus; and
WHEREAS, the voluntary incentive will help the City to verify vaccination status of employees
prior OSHA's COVID-19 mandate.
NOW, THEREFORE, BE IT RESOLVED that the City Council approves the Voluntary
COVID-19 Vaccination Incentive Program.

Voluntary COVID-19 Vaccination Incentive Program
The City of Lakeville is committed to providing a safe and healthy workplace for all our
employees. The CDC's most recent guidance states that getting vaccinated prevents
severe illness, hospitalizations, and death.
The City of Lakeville is pleased to announce an incentive for employees to take the
necessary steps to become fully vaccinated against the COVID-19 virus. This incentive
will provide up to 8 hours of paid time off after a regular full-time or part-time employee
provides proof of full vaccination. This will also apply to members of bargaining units if
approved by the exclusive representative in a Memorandum of Understanding. The paid
time off incentive must be used by March 31, 2022.
The incentive for paid -on-call firefighters, part-time sales associate employees, and Art
Center non -benefited employees will be a $100 one-time payment that will be made after
the employee provides proof of full vaccination. The one-time payment is a taxable
benefit.
The following are details of the program:
1. The incentive program applies to regular full-time employees, regular part-time
employees, paid -on-call firefighters, part-time sales associate employees, and
Art Center non -benefited employees.
2. Full-time benefit earning employees will receive 8 hours of paid time off to be
used by March 31, 2022. Part-time regular employees' hours will be prorated
based on average hours worked. The paid time off will be must be used by
March 31, 2022.
3. The program will be available through December 30, 2021 and is retroactive to
include all previously fully vaccinated employees.
4. In order to receive the incentive, employees must provide a copy of their
vaccination record and complete a Voluntary COVID-19 Vaccination Incentive
form to Human Resources by 12:00 p.m. on Thursday, December 30, 2021.
Employees will also be required to sign a Tennessen Warning. Employees who
do not submit proof of vaccination by 12:00 p.m. on Thursday, December 30,
2021 will not qualify for this incentive.
5. Employees who choose not to be vaccinated will not be eligible for this
voluntary incentive.
