RESOLUTION AUTHORIZING REIMBURSEMENT FOR USE OF PERSONAL
AUTO ON CITY BUSINESS AND ESTABLISHING THE POLICY,
REQUIREMENTS AND CONDITIONS FOR THE CLAIM
WHEREAS, it is deemed to be beneficial to the City of Lakeville that City Personnel used
their personal vehicles on City business, and
WHEREAS, the City Council believes it to be equitable and reasonable to reimburse the
employee for the use of their personal vehicle while on City business.
NOW THEREFORE BE IT RESOLVED, by the City Council of Lakeville, Minnesota,
that the following policy, conditions and requirements are established.
1. Claims shall be submitted to the Finance Director on a claim form provided by the City
and shall be itemized, showing the date, destination, purpose of the trip and mileage, and
be signed by the person making the claim.
2. The mileage rate for reimbursement effective April 1, 1999 shall be $.31 per mile.
3. Employees using their personal vehicle and claiming mileage reimbursement on City
business shall assume liability through his/her own vehicle insurance carrier for personal
injury, property damage and comprehensive/collision damage to their vehicle.
4. Mileage reimbursement shall be made upon City Council approval of the claim.
5. The "Use of Vehicles" section of the Employee Personnel Policy is amended accordingly.
APPROVED AND ADOPTED by the City Council of the City of Lakeville, Minnesota,
this 1st day of February, 1999.

USE OF VEHICLES
POLICY
1. An employee on City business is required to use a City vehicle if possible. All use
of City vehicles must be approved by the immediate supervisor.
2. An employee using a City vehicle must have a valid driver's license in the
appropriate class. Any violation of this provision shall subject the employee to
disciplinary action up to and including termination.
3. Employees using City vehicles must be particularly mindful of all traffic
regulations and courtesies of the road. Abuses and violations may subject the
employee to disciplinary action, up to and including termination.
4. Unless approved by the City Council, the use of City vehicles for personal reasons
is prohibited. Violation of this provision may subject the employee to disciplinary
action.
5. Use of a private vehicle for City business must receive prior approval from the
immediate supervisor. The mileage rate for reimbursement effective April 1, 1999

the responsibility of the employee to satisfy. Use of the employee's private vehicle
for City business must also meet requirements of the U.S. Internal Revenue Service
as to the reporting of claims for mileage paid by the City.
6. An accident while on City business with either a City vehicle or private vehicle
shall be immediately reported verbally to the supervisor and to the Administration
Department. This initial report shall be followed up with completion of the
required insurance carrier form and pertinent incident reports as soon as possible.
