RESOLUTION MODIFYING THE EMPLOYEE HANDBOOK
CONCERNING EMPLOYEE INSURANCE
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of
Lakeville that effective January 1, 2000 the policy on page 36 of the Employee Handbook,
"Employee Insurance" dated December 21, 1998, is hereby rescinded and replaced with
"Employee Insurance" dated November 15, 1999.
ADOPTED by the City Council of the City of Lakeville this 15th day of

Policv:
1. Effective January 1, 2000, the EMPLOYER shall pay for 100% of the cost of single
coverage, or up to the following monthly maximum, per EMPLOYEE, toward the cost of
group medical family insurance.

2. Waiver of Coverage:
Effective for 2000, the employee may elect to waive health insurance coverage..
Employees who waive coverage will receive compensation at the rate of $50 per month.
. Annual re-certification of alternate health insurance coverage will be required from all
employees who elect the waiver option.

DENTAL INSURANCE
Effective January 1, 1999, the employer shall pay 100% of the cost of single coverage for
Group Dental Insurance. If the employee elects additional dependent coverage, the employee.
will pay the difference between the cost of single coverage and the cost of family coverage.
This cost shall be paid by the employee via payroll deduction. Participation in dental coverage
is voluntary.
LONG TERM DISABILITY INSURANCE
POLICY
1. Long-term disability insurance shall be provided through Fortis Benefits Insurance
Company to:
a) Permanent, full-time employees;
b) Permanent, part-time employees working thirty (30) hours or more each week.
2. The City shall contribute one hundred percent (100%) of the cost of the premium fora ,
long-term disability insurance policy for all eligible employees.
3. Employees covered by a collective bargaining agreement shall be covered by the
collective bargaining agreement in regard to long-term disability insurance benefits, if
any.
