RESOLUTION MODIFYING THE EMPLOYEE HANDBOOK
CONCERNING EMPLOYEE INSURANCE
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of
Lakeville that effective January 1, 1999 the policy on page 36 of the Employee Handbook,
"Employee Insurance" dated December 1, 1997, is hereby rescinded and replaced with
"Employee Insurance" dated December 21, 1998.
ADOPTED by the City Council of .the City of Lakeville this 21st day of
December, 1998.

2. Permanent part-time employees working 30 or more hours each week.
Policy:
1. Effective January 1, 1999, the EMPLOYER :shall pay for 100% of the cost of single
coverage, or up to the following monthly maximum, per EMPLOYEE, toward the cost of
group medical family insurance.

DENTAL INSURANCE
2. Effective January 1, 1999, the employer shall pay 100% of the cost of single coverage for
Group Dental Insurance. If the employee elects additional dependent coverage, the
employee will pay the difference between the cost of single coverage and the cost of family
coverage. This cost shall be paid by the employee via payroll deduction. Participation in
dental coverage is voluntary.
