JUNE 21, 2010
Mayor Dahl called the meeting to order at 7:00 p.m. The pledge of
allegiance to the flag was given.

Presentations /Introductions
Chief Vonhof introduced Susan Long, Director of Clinical & Support
Services with Alllina Medical Transportation, who presented Golden Stork
certificates and pins to several individuals involved in an emergency
childbirth. Certificates and pins were presented to Police Officers Kevin
O'Neill, Jeff Hanson and Chris Gartzke, Allina paramedics Casey
Haggstrom and Tom Svihel, dispatcher Mindy Kaltenhauser and the
baby's father, Michael Gunderson.
Ms. Long presented the Allina Medical Transportation 2009 Annual Report.
Pan -O -Prog President Jim Dunn presented 2010 Pan -O -Prog buttons to the
Mayor, Council members and City staff.
Chief Vonhof presented the April /May Police Department monthly report.
John Hennen, Park Maintenance & Operations Manager, presented the
May monthly report of the Parks & Recreation Department.

Consent Agenda
10.31 Motion was made by Miller, seconded by Rieb to approve the consent
agenda items as follows:
a. Claims for payment.
b. Minutes of the June 7, 2010 City Council meeting and the May 24,
2010 Council work session.
C. Resolution No. 10 -42 appointing election judges for the August 10,
2010 State Primary Election.
d. Resolution No. 10 -43 appointing Advisory Board for Downtown
Special Service District No. 1.
e. Contract with Pan -O -Prog relating to insurance.
f. Site lease agreement with TTM Operating Company for the Fairfield
Water Tower.
g. Joint Powers Agreement with Dakota County Multi- Agency
Assistance Group.
h. Resolution No. 10 -44 authorizing the use of certain parks, Senior
Center and Arts Center for Pan -O -Prog events.
i. Agreement with RES Specialty Pyrotechnics for the service of an
outdoor fireworks display.
j. Acceptance of improvements and final payment to Mickman
Brothers, Inc. for North Park Field Irrigation, Improvement Project 09-
06.
k. Tree work license for Arbor Doctor.

Avenue.

district, located at 16105 Cedar Avenue.

Roll call was taken on the motion. Ayes, unanimous.
Item 7 - Sawa Japan MN, Inc. On -Sale and Sunday Liquor License
Mayor Dahl opened the public hearing on the application of Sawa Japan
MN Inc. for an on -sale and Sunday liquor license at Sawa Japan
Steakhouse and Sushi, 18453 Orchard Trail. The City Clerk attested to
legal notice publication.
The applicant, Zhong Qin Zheng, requested approval of an on -sale and
Sunday liquor license for his new restaurant being constructed in the
Timbercrest development. Sergeant Kornmann presented the police
report and recommended approval of the license as requested. There
were no questions or comments.
10.32 Motion was made by Bellows, seconded by Swecker to close the public
hearing. Voice vote was taken on the motion. Ayes, unanimous.
10.33 Motion was made by Swecker, seconded by Rieb to grant an on -sale and
Sunday liquor license to Sawa Japan MN Inc., d.b.a. Sawa Japan
Steakhouse and Sushi, 18453 Orchard Trail, subject to receipt of a
certificate of occupancy from the City of Lakeville Building Inspector.
Roll call was taken on the motion. Ayes, unanimous.
Item 8 - Lakeville Capital Investments On -Sale and Sunday Liquor License
Mayor Dahl opened the public hearing on the application of Lakeville
Capital Investments, Inc. for an on -sale and Sunday liquor license at
Goodfellas Pizza Bar & Grill, 20643 Kenrick Avenue. The City Clerk attested
to legal notice publication.

Sherry DeBough, Manager of Goodfellas Pizza Bar & Grill, stated their
restaurant currently holds a wine and 3.2 beer license, and they are now
requesting a full liquor license. Sergeant Kornmann presented the police
report and recommended approval of the license as requested. There
were no questions or comments.
10.34 Motion was made by Rieb, seconded by Miller to close the public
hearing. Voice vote was taken on the motion. Ayes, unanimous.
10.35 Motion was made by Rieb, seconded by Swecker to grant an on -sale and
Sunday liquor license to Lakeville Capital Investments, Inc., d.b.a.
Goodfellas Pizza Bar & Grill, 20643 Kenrick Avenue.
Roll call was taken on the motion. Ayes, unanimous.
Item 9 - Liquor License Renewal
Mayor Dahl opened the public hearing on the renewal of on -sale liquor,
Sunday liquor, wine and 3.2 beer licenses. The City Clerk attested to legal
notice publication. Sergeant Kornmann reported that all applicants have
met the requirements for renewal. Holiday Stationstore #504 did fail a
compliance check earlier this year. A recommendation on the civil
penalties for Holiday Stationstore will be forwarded to the City Council at
a later date. There were no questions or comments.
10.36 Motion was made by Bellows, seconded by Miller to close the public
hearing. Voice vote was taken on the motion. Ayes, unanimous,
