RESOLUTION AMENDING THE LAKEVILLE LIQUORS
AGE VERIFICATION POLICY
WITH THE CITY OF LAKEVILLE, LIQUOR DEPARTMENT
WHEREAS, the City Council has, from time to time, amended by resolution the.
policies to be observed by the employees of the City of Lakeville, Liquor Department.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of
Lakeville:
1. The Lakeville Liquors Customer Identification Policy, (pages 62 and 63 of
the Lakeville Liquors Employee Manual), is hereby amended and adopted
for all positions of employment with the City of Lakeville, Liquor
Department.
2. All previous resolutions and motions ado ted b the Cit Council with

provisions similar to the provisions in this Lakeville Liquors Amended
.Customer Identification Policy as attached to this resolution are hereby
repealed.
3. The amended policy will take effect July 8th, 2003
APPROVED AND ADOPTED by the City Council of the City of Lakeville,
Minnesota, this 7th day of July, 2003.

CUSTOMER IDENTIFICATION
(Amended 7-7-03)
POLICY
The Lakeville Liquors primary objective is to sell alcohol and tobacco products
legally and responsibly. Every individual who desires to `purchase any item of
Lakevil a Liquors must produce legal identification prior to the sale occurring. As
policy, Lakeville Liquors will sell no products including non-alcoholic products
such as pop, tobacco, or ice to anyone who is not of legal drinking age (21 years
of age). As .policy, all individuals withi a party must present identification upon
request. Selling alcoholic beverages to an underage individual is punishable by
law under Minnesota State Statute 340A.503.
Forms of Identification Accepted:
Lakeville Liquors will accept only the following forms of identification:
1. Valid photo Minnesota Drivers License
- License must be valid.
- Paper license application. must accompany all clipped drivers
license.

2. Valid photo Minnesota Identification Card
- Identification must be valid.
- Must provide one (1) otherform of identification.
3. Valid photo Drivers License from another state may be accepted by only
keyholders, store supervisors or the director.
Identification must be valid
- Comparison must be made with Out-of-State Identification
Guide kept at each store location.
- Identification must match book in every detail.
4. Valid. upgraded 2002 formatted Military Identificaion
- Identification must be valid
5. Valid Military Identification printed on Department of Defense printed card
stock
- Identification must be valid
- Must provide one (1) other form of identification

Identification Verification:
Once identification has been provided, al( details of the document must be
verified ensuring that identification matches the persoh in possession of it, and
that the identification .has not been altered. in any manner. In the event that you
discover what you believe is a false for altered identification, or if a minor is
attempting to purchase complete the following process:.
1. Withhold the identification from the customer.
2. Call and notify the police 985-4800, inform the ;dispatcher whether the
person is waiting or has left the premise.
3. Ifthe customer agrees, have them wait for police verification. ' Do not ever
place yourself or others in jeopardy by attempting to detain the customer.
4, If the customer flees the building, document the following information o an
incident report immediately:
a. Vehicle make, model and description
b. Vehicle license number '
c. Description of the suspect
d. Direction .the suspect headed
5. Release identification to the responding officer.
6. Document all information on an incident report.

i acknowledge receipt of he Lakeville Liquors Identification Policy as it pertains
to the sale of alcohol and tobacco products.. I have read the policy and have been
able to ask my supervisor questions about any part of~the policy I do not
understand. i further understand that consequences for non-adherence to this
policy may constitute just cause for discipline including,verbal reprimand, written
warning or termination.
