BE IT RESOLVED by the City Council of the City of Lakeville that effective January 1, 2004, the policy entitled."Employee Insurance" is approved and replaces pages 57, 58 and 59 in the Employment Benefits section of the Employee Handbook as described in Schedule A attached. This change reflects the taxation method ofthe Long Term Disability Insurance. BE IT .FURTHER RESOLVED the Fortis Long Term Disability Plan as described in Schedule B attached hereto is hereby approved. • APPROVED this 20th day of October 2003, by the City Council of the City of Lakeville. Full-time employees will be eligible to receive life insurance coverage under this policy. , b. Part-time employees working 30 hours or more each week also will be eligible to receive life insurance coverage under this policy. c. The above referenced full-time and part-time employees. will be covered on the first day of the month following employment with the City. 2. Policy & Coverage • The City will contribute 100% of the cost of the premiums fora $15,000 term life insurance policy for all eligible employees. The policy provides coverage for the employee and dependents according to the following classification as per terms of the City's policy with Commercial Life Insurance. Amount of Insurance Class Term Life and AD&D 1) Employee $15,000 2) Spouse $ 1,000 3) Unmarried Child(ren) $ 1,000 except that dependents under age 14 days to 6 months are insured for only $100 HOSPITAL, MEDICAL AND SURGICAL INSURANCE 1. insurance Provided. The City Council by resolution will establish from time to time the insurance plans that will be provided. • 2. Eligibility a. Part-time employees working 30 or more hours each week also will be eligible to participate in the insurance plans available under this policy. c. Policy & Coverage The City Council by resolution will establish from time to time the costs the City will pay for employee and dependent coverage under City group hospital, .medical, and surgical insurance programs. DENTALINSURANCE 1. Eligibility a. Full-time .employees will be eligible to receive dental insurance • coverage under this policy.. b. Part-time employees working 30 hours or more each week also will be eligible to receive dental insurance coverage under this policy. c. The above referenced .full-time and .part-time employees will be covered on the first day of the month following employment with the City. 2. Policy & Coverage The City Council by resolution will establish from time to time the costs the City will pay for employee and dependent coverage under City group dental insurance programs. LONG TERM DISABILITY INSURANCE 1. Eli ibilit a. Full-time employees will be eligible to participate in the insurance plans available under this policy. M b. Part-time employees working 30 or more hours each week also will be eligible to participate in the insurance plans available under this policy. c. The above referenced full-time and part-time employees will be covered on the first day of the month following employment with the City. 2. Policy & Coverage The City shall contribute one hundred percent (100%) of the cost of the premium for along-term disability insurance policy for all eligible employees. The value of the premium is taxable to the employee. The employee is responsible for the applicable FICA, Medicare .State and Federal Taxes on the value of the premium.
